Application for Para Drivers for TEAM USA World Championships at Sandringham, England June 27-29, 2014
Dear USDFD Members,
USEF has posted the application and forms which need to be submitted if you would like to be considered for Team USA this year. The competition is in Sandringham, England from JUNE 27 – 29, 2014. The deadline is very tight. Please read the following in its entirety:

REQUIRED DOCUMENTS for Drivers (More if shipping a horse): – The following documents are required to be properly executed as a condition of nomination in the Event and are at the main link above:
• Application (Attachment A)
• Horse Loan Agreement (Attachment B)
• USEF Code of Conduct (Attachment C)
• USEF Release, Assumption of Risk, Waiver and Indemnification (Attachment D)
• Declaration of Scores (Attachment F) **
**Currently the link for Attachment F – Declaration of Scores Form is not correct. This has been noted and should be corrected shortly. In the meantime I would simply submit my scores as described in the Selection Document: overall competitive records and scores in the Dressage competitions, penalties obtained in the Marathon, accuracy and speed in the Cones competition. “The athlete must submit Scores from any FEI, USEF and/or ADS Recognized Combined Driving Event at the Preliminary Level or higher – see 3f in ‘2014 Selection Procedures’ pdf.”

Applications submitted online on or before March 31, 2014 will be charged $70 per Application. Applications submitted online after April 11, 2014 and received by the USEF on or before April 11, 2014 will be charged $300 per Application. NO APPLICATIONS WILL BE ACCEPTED AFTER APRIL 11, 2014.
Selection to the Team does not guarantee that funds will be available from USDFD or USEF to send a team.
